If a file has a RCS cache "file,v" and another "RCS/file,v" we
should find and complain better.  We take the toplevel one last
and overwrite the RCS/file,v's output.  This is the opposite of
what I want.  This should be a fatal error (#33).

Preserve the +x bits on files co'd (-m does it now).
